When contacting SAILS with a problem, there are often standard pieces of information we need. The best way to be sure to include what we need to solve a problem is to take a look at the forms listed below. They will help guide you though submitting what we need.
- General problem form - covers most everything not cataloging related.
- Circulation Problem form - for issues specific to circulation.
- Cataloging Error Reporting Form - for specific cataloging problems
- Overdrive Card Replacement - If a patron replaced their library card and has outstanding Overdrive transactions, use this form to request their old card be merged with their new one.
